Car accident care · Who pays · San Antonio

Will treatment cost me anything?Start here: your first visit is $0.

You're hurting, there's already a bill from the ER, and you're doing insurance math at the kitchen table at midnight. Here's the short answer, before anything else.

Your injury evaluation costs you nothing out of pocket. We bill the coverage responsible for your accident — your PIP, the at-fault driver's insurance, or your attorney's letter of protection — and Silvia, our practice manager, walks you through the rest before you leave.

The short answer

Three things to know before you worry about the rest.

  1. 1. The first visit is $0 out of pocket.

A hands-on exam, and most accident patients get their first treatment that same visit. Nothing is due at the front desk.

  2. 2. After a wreck, auto coverage is built to go first.

PIP and the at-fault driver's insurance exist for exactly this — care after a crash. We bill them for you.

Sorting out the coverage is our job, not yours.

  3. 3. If you have an attorney, your care goes forward without you paying up front.

We accept letters of protection, with no special conditions.

And one person handles the details. Silvia figures out which coverage applies to you, explains it in plain English, and keeps the paperwork moving so you can focus on getting better.

Who pays, in plain English

Four kinds of coverage. Here's what each one means for you.

This is general information about how accident care is usually covered in Texas, not legal advice. Silvia works out which of these applies to you.

PIP — Personal Injury Protection (your own auto policy)

What it is: coverage on your own car insurance for medical care after a crash.

What it means for you: Texas auto policies include at least $2,500 per person in PIP unless the policyholder turned it down in writing — so there's a good chance you have it. It pays regardless of who caused the crash. A lot of people find out they've been carrying it for years.

The at-fault driver's liability insurance

What it is: the other driver's coverage for injuries they cause.

What it means for you: if the other driver caused the wreck, their insurance is ultimately responsible for your accident-related care. We see auto cases every week and handle that billing for you.

Worth knowing: that insurer represents the other driver, not you. Depending on the carrier and how serious your injuries are, they can start pushing back on a claim early. If that happens, Silvia tells you right away and walks you through your other options, like your own PIP or a letter of protection if you have an attorney.

A letter of protection (if you have an attorney)

What it is: a letter from your attorney to us, putting in writing that our bill will be handled through your case.

What it means for you: your care goes forward without you paying up front. We accept letters of protection with no special conditions. More on how it works →

Your health insurance

What it is: the plan you already use for everything else.

What it means for you: it can come into play too. Auto coverage is usually responsible first after a wreck; Silvia sorts out which coverage applies to you, including your health plan when it's the right one to use.

Your situation

Find yours. The answer is shorter than you think.

"I have an attorney."

We accept your attorney's letter of protection, with no special conditions, and your care goes forward without you paying up front. Bring your attorney's name and phone number to your first visit.

"I don't have an attorney, and the other driver caused it."

You don't need one to be treated here. Your first visit is $0 out of pocket; after that, your PIP and the other driver's insurance are the coverage built for this, and we handle that billing for you. Bring the claim number if you have one.

"The crash was my fault — or nobody's sure yet."

You can still be treated, and your first visit is still $0 out of pocket. PIP pays regardless of who caused the crash, and your health plan can come into play too. Silvia works out what applies.

"The other driver had no insurance — or drove off."

Call anyway. Your first visit is $0 out of pocket, and your own PIP pays regardless of who caused the crash. Your own auto policy may also include uninsured motorist coverage, which can cover expenses related to your injuries. Silvia will look at every coverage that could apply to you, including your health plan.

Letters of protection, up close

An LOP means your care doesn't wait on your case.

If you have an attorney, a letter of protection is the simplest way to get care started. Here's how it works here:

  • Your attorney sends us the letter. It puts in writing that our bill will be handled through your case, so you don't have to cover it visit by visit.
  • We accept it, with no special conditions.
  • Your care goes forward without you paying up front. Exam, treatment, and imaging when your exam calls for it.
  • Silvia walks you through it before you leave your first visit — how your account works and where things stand — so nothing about it is a mystery.
Your attorney is your choice — or no attorney at all. We never refer patients to lawyers. If you have one, we work with them. If you don't, you're just as welcome, and nothing about your care changes.

What Silvia does for you

You bring what you have. Silvia sorts out the rest.

You don't need to know your coverage before you call. Most people don't. Bring whatever you have, and Silvia takes it from there:

  • Your driver's license and your auto insurance card
  • The claim number and the other driver's insurance information, if you have them
  • Your attorney's name and phone number, if you have one
  • Your health insurance card
  • The police report number, if you have it

Plan on about 90 minutes for your first visit, treatment included. You'll get a hands-on exam; if your injury calls for medication, our nurse practitioner can see you that same day, in the same building. Before you leave, Silvia explains which coverage applies to you in plain English.

Standards that protect you

The money question is real. It isn't the first question.

After a wreck, the thing to solve first is what's actually injured. These are our standards, and they're the reason you can come in before you have every answer:

  1. Exam first, paperwork second. You're examined before anything else happens, and Silvia explains your coverage before you leave.
  2. A reason for every visit. Each one is written in your chart, and your plan changes as you improve.
  3. One building, one record. Chiropractors, a nurse practitioner and a pain management physician, Dr. Praful C. Singh, M.D., work in the same building, with imaging arranged by us — so your records tell one complete story.
  4. Your choice of attorney — or none. We never refer you to a lawyer.
  5. Released when you're better. Treatment ends when your exam says you've recovered or reached maximum medical improvement, not when a visit count runs out.

ONE MORE THINGGetting examined isn't "building a claim." It's refusing to carry someone else's mistake in your neck for the next ten years.

Questions patients actually ask

Fair questions

Will I pay anything at my first visit?

No. Your injury evaluation is nothing out of pocket. We bill the coverage responsible for your accident, and Silvia walks you through where things stand before you leave.

What does the no-cost evaluation include?

A hands-on exam — range of motion, which movements provoke pain, reflexes and strength. Most accident patients get their first treatment that same visit, and if you need medication, our nurse practitioner sees you before you leave. Plan on about 90 minutes.

Do I need to know my coverage before I call?

No. Bring what you have — your insurance card, a claim number if you have one — and Silvia works out which coverage applies to you.

What is PIP, and do I have it?

Personal Injury Protection is part of your own auto policy. Texas policies include at least $2,500 per person unless the policyholder turned it down in writing, and it pays regardless of who caused the crash. Many people have it and don't know.

Will my health insurance be used?

Auto coverage — PIP and the at-fault driver's insurance — is usually responsible first after a wreck. Your health plan can come into play too, and Silvia sorts out which coverage applies to you.

What is a letter of protection?

A letter from your attorney to us, putting in writing that our bill will be handled through your case. We accept them with no special conditions, and your care goes forward without you paying up front.

Do I need an attorney to be treated?

No. That decision is yours, and we don't give legal advice — we're doctors. If you have one, we work with their letter of protection. If you don't, you're just as welcome here.

Will you refer me to a lawyer?

No. We never refer patients to lawyers. Bring yours, or come without one.

The accident was my fault. Can I still come in?

Yes. Your first visit is $0 out of pocket, PIP pays regardless of who caused the crash, and Silvia works out what else applies.

What if I need an MRI?

If your exam or your symptoms call for one, we order it, arrange it and handle the paperwork, and Silvia explains how it's covered for you. If imaging shows a disc injury, decompression on the DRX9000 is down the hall.

Can I come in today?

Monday through Thursday, yes — we see new patients the same day, and no referral is needed. Call (210) 741-9166.
